This would mean focussing on the individuals’ needs, likes, history, choices and not their disability. Person centred care planning is an essential part of this and It should be created with them. Explain the importance of recognising the centrality of an individual rather than the disability.By recognising and considering the centrality/needs of an individual, you are solving the difficulties of the individual rather their disability. Working in partnership with the individual and considering them as a whole will strengthen their dignity and self-respect, also boost that individual’s confidence and self-esteem. This will also allow us not to misjudge the individuals own capability and capacity by purely focusing on their disability.   Explain person centred care. The term ‘person-centred care’ is used to refer to many different principles and activities; there is no single agreed definition of the concept. Person-centred care aims to ensure a person is an equal partner in their health care.
