My Client Profile
My clients name is Jayjay Singh. I have changed his name for confidentiality reasons. Jayjay is a 73 year old male, so he is in the stage of later adulthood. He has two children, and his wife died just over a year ago from a natural cause.
My Clients Health Needs
Jayjay has recently discovered that he is suffering from heart failure. This means that his heart isn't able to pump blood round the body as well as it should. He also has respiratory problems, where is unable to breath properly and gets very tired and weakened easily.

My Clients Health Need 1
Jayjay was diagnosed with heart failure 4 months ago. He suffered from dizziness and found he got out of breath and tired very easily, he also had rattles in his lungs; these are some of the symptoms that lead to heart failure. Jayjay goes to see his GP once a week for general check-ups, e.g. check his blood pressure and to see if he still had breathing problems.
Jayjay will also see a dietician to help him improve with the condition, in his eating habits which again he will see once a week. This is important because if lifestyle changes are not made, it could worsen the condition than it already is.
Jayjay will also see a Hospital Doctor who has medical knowledge and skills on the diagnosis, prevention and management of the disease. As well as treating the patient the Hospital Doctor may refer him to a wide range of specialists including; nurses, radiographers, pharmacists and physiotherapists, which Jayjay will come into contact on a number of occasions. He will see his pharmacist when he goes to pick up his prescriptions and medication provided by a Hospital or Medical Doctor who he will see twice a week.
Jayjay is still undergoing investigation to the cause of his condition, Doctors are still unsure of which part of the heart is affected and Jayjay will be soon getting his results in two months.

My Client Health Need 2
My client’s health second health need is his Respiratory problems. This is where he is unable to breathe properly and tires and weakens very easily. He went to his GP which took a physical test, taking my clients family health history. He also did a simple breathing test to see how much he can blow out of his lungs and how fast he can blow it to determine the amount of airway obstruction. Jayjay finds it hard to get on with his daily life. Even going up the stairs is a struggle for him. The specialists still do not know what is causing the Respiratory problems, and do not know whether it is asthma, or another cause. So doctors a overlooking the history of his family health, to see whether there could be a conclusion for his suffering.
He will see his GP about his condition and so that his GP can do some tests to see whether the treatment Jayjay is receiving is working.
Due to Jayjay’s condition, his daughter, Melissa, comes over to help round the house and assist to his daily needs, e.g. cleaning and cooking, because Jayjay is unable to do this himself.
